Enemies shoots through walls, one shot in the back kills you, not only in the head. Boss on Chapter 22 hits me through the boxes. Just unfair. Dispose this awesome poor game now

But it's supposed to be hard anyways. I think it was a bit dumb how they made U2 easier (on crushing. Compared to UDF.)
And..I'm not sure you can call it unfair...Because the AI don't care, if you get what I mean.

Anyways, if you're having trouble, take a break, look for a walkthrough on youtube! They can be very helpful.
By "Extreme Heavy Mode" I think you're referring to Crushing Mode. Yes, its difficult because you get killed by three shots from enemies, but not impossible. You need to have potential skill and luck on fighting off waves of enemies who are shooting at you and throwing grenades at you. If you spot a group of enemies and are gathered in one place, you can throw a grenade at them, which then minimizes the number of enemies. Oh yeah, and getting headshots tends to be tricky sometimes as they move around so timing is crucial.
As for Chapter 22... you just need to try harder. You need to time your attacks right against Navarro or he'll just kill you in a matter of seconds if timing your attacks incorrectly.
EDIT: Just a minor edit, I don't recall enemies being capable of shooting through walls and hit you. Maybe you're referring to a Call of Duty game?
