Same exact story with me. I can log in everywhere except on my PS4, and my girlfriend's account, on the same PS4, works! **bleep** it ;/
Have you solved the issue? If not, maybe it;'s worth trying to deactivate it on SEN website. You can't deactivate one specific console, HOWEVER, If you go to Dashboard/Account Details/Media and Devices -> Deactivate All
It might work. At the moment I'm backing up my files before trying to delete the account, so I can't tell yet if it helps.

Contact your Internet Service Provider (ISP) and request them to open the following ports in the router/modem’s settings:
TCP Ports: 80, 443, 1935, 3478-3480. 3659, 10050, 10051, 10000-10099, 10040 – 10060, 42127
UDP Ports: 3074, 3478-3479, 3659, 6000, 50000 – 60000
¨ Request them to update the router/modem’s firmware.
¨ Ask if it’s possible to disable the router/modem’s Firewall.
¨ If possible use a wired connection, request your ISP to help you set a static IP and they should be able to add it to the DMZ of your router/modem.
¨ The Router/Modem may be configured to "filter" MAC addresses, on this case you will need to access the router and input the MAC address of your console.
