Solution is three posts above from UJ. Since this thread was created there's now a CS page that helps out with these kinds of issues.
Also, pretty sure that moderation left the address because it's an example, not the real address.

This is unbelievable. The fact that this thread started in 2008 and people are still having this problem in 2013 is just unfathomably stupid to me. As someone who is switching to Sony after Microsoft's fiasco, this is an unsettling first experience. Sony is a behemoth, this should have been fixed. They've had 5 years.
My solution to it was this:
1) Enter the information through your online account on the computer, not the console.
2) Enter the billing address EXACTLY as it appears on your CC statement. Do not check the "use residential address". Sony's payment servers pull word for word what the credit card statement says. Once I did that it worked.
Really stupid problem to ha ve, but those 2 steps seemed to solve it.
