Small and Wide is a matter of luck, but just keep starting at the beginning of Level 3 and you'll get it. Powerups are dropped randomly, but both small and wide can be earned there.
If you're good at the last boss on Level 2, you might start there, as Wide is a fairly common drop on that level.
So I logged in to PS Home, and lo and behold a reward. What was it? The Xevious Resurrection cabinet . Without getting the last 2 stamps.
So apparently the criteria has changed, or people were mistaken on what was needed.
I have it @ only 23% completion.
I think it takes just 4 stamps each game..?
Alphazone4.com lists it as a reward for unlocking every Namco Essentials stamp.
I know I have at least four stamps in every game, and I still don't have it.
