I have posted this same problem in the COD4 section of these forums, and the problem is real. But for some reason on this thread most of you think it's not a COD problem but the user’s internet instead. In the COD section almost everyone who responded to this issue can relate to it from time to time, they blame it on the servers. They call it Server Lag and Inaccurate Hit Detection. To me it just seems like the game is not representing everyone the same in real time regardless of who is to blame. Hopefully as the internet keeps getting better and the games get better, this will be less of a problem. I believe in the next patch, this issue may be helped.
I have 7 mbps speed as well, and I see the kill inaccuracies happen at least once a game out of the 6-7 times a game I get killed. But I can also tell from time to time that I kill someone else who I think maybe should have killed me first. I just say to myself, "well that dud it’s going to be pissed when he sees his kill cam."
On a happier note, the game is so dead on in every way, we just never notice such detailed inaccuracies in games until now. The next gen game has raised our game play online, but has more defined detailed problems in gaming within our internet technology. Think about it, it wasn't too long ago our internet wouldn't even have been able to make this game happen online. Our speeds have increased 5 times in 5 years. But it still isn't perfect. Maybe in another 5 years it will be. But I am sure new, even more detailed problems will arise. But even with the current issues, this game is incredible and I have spent more hours with this game than any other in my life and I am 41 years old.
